Granite VNA provides community services for infants through seniors in preventative health care, health education and health maintenance. Special programs are available, such as Baby's First Homecoming for new mothers who are discharged from Concord Hospital, and foot care clinics that offer foot care, blood pressure screenings, medication education, and nutritional counseling to seniors.

Mobile health teams bring medical and social work services to places like shelters, soup kitchens and public housing. Services provided at clinics include preventive health screenings; sick visits; and chronic-disease management. The team will also link patients with staff at Families First and Goodwin Community Health who can help with eligibility determination and applications for housing assistance, food stamps, WIC, TANF, disability benefits, Medicare, Medicaid, other health insurance, veterans benefits and more. Clinic staff can also help patients access substance misuse and mental health counseling, family support, specialty care, medications, and labs and diagnostic testing. ** As of September 2024, clinics were offered in Dover, Rochester, Portsmouth and Exeter. -The Nashua Community Health Department offers clinics for various services including Blood Pressure and STD/HIV/HCV. -All childhood immunizations are available for individuals who are 18 years and younger. A parent or legal guardian must accompany minors under age 18. -Immunizations are available to adults over the age of 18 who have no insurance or are under-insured, which means you have health insurance, but the coverage does not include vaccines or the insurance only covers selected vaccines. -Other vaccines available include: Flu, Tetanus, and Shingles.

The Health Care for the Homeless program provides primary medical care, nursing case management, addiction counseling, eye care, mental health care, dental care, and health education to homeless people in Manchester, regardless of their ability to pay. Nurse case managers guide patients through the health care system, helping them to also to obtain social services and housing. HCH has a mobile medical van that goes to Manchester shelters and parks.
