NH Healthy Families offers health insurance plans to those on NH Medicaid. Programs eligibility depend on age, income, family size and any special health needs.

AmeriHealth Caritas New Hampshire provider health insurance coverage for NH Medicaid. Most NH Medicaid Program recipients receive health care services through the Medicaid Care Management (MCM) program. These Health Plans cover the same services that NH Medicaid does. The Health Plans also offer additional optional services, programs, and other extras to promote healthy behavior. Extra benefits include adult dental, car seats, WW (formerly Weight Watchers) and Mission GED, where eligible members can earn their high school equivalency.

New Hampshire Family Voices provides free, confidential services to families and professionals caring for children with chronic conditions and / or disabilities Services include: Individual assistance by telephone Assistance navigating through systems (health, human services, education, and health care financing). Assisting families to make more informed decisions about their children’s health care and be better able to participate in systems building activities in their communities and within NH Information and resources Trainings for families on healthcare, financing, best practices and family/professional partnerships. Well Sense Health Plan is a not-for-profit managed care organization (MCO). We are a health plan selected by the State of New Hampshire to offer high-quality healthcare coverage to Medicaid enrollees throughout the state. We provide state-wide access to primary care physicians, specialists, behavioral health, emergency room services and hospital care for our members.

Our Case Managers offer free, one-on-one support to patients and families to help them solve real-life problems related to a serious health condition.
