Granite VNA provides community services for infants through seniors in preventative health care, health education and health maintenance. Special programs are available, such as Baby's First Homecoming for new mothers who are discharged from Concord Hospital, and foot care clinics that offer foot care, blood pressure screenings, medication education, and nutritional counseling to seniors.

We provide comprehensive virtual care for adults with Pre-diabetes, Type 2 diabetes, Obesity, Hypertension, and/or High Cholesterol. Patients can track their blood sugar levels, medication adherence, and symptoms at home, while their clinician can review the data, adjust treatment plans, and offer personalized advice remotely. All care is coordinated and communicated directly to the patients primary care team. We are an extension of the primary care team and partner with both the patient and primary clinician to help you manage your health. Services include: - Weekly, biweekly, monthly, or quarterly telehealth visits tailored to your individual needs. - Medication and Insulin Management. Includes prior-authorization handling for related medications. - Continuous glucose monitoring. - Nutrition & lifestyle counseling. - Remote patient monitoring (Blood sugar, Blood pressure) - Chronic care management. - Coordination of Diabetic Retinopathy Screenings & Diabetic Foot Exams. - Limited availability for home visits - determined on a case by case basis.

Medical services provided include but not limited to: IV therapy, wound care, medication management, injections, baby wellness visits, pain management, occupational, physical, speech therapies. Other services provided include but not limited to: personal care, ostomy care, catheter care, respite, bathing, meal prep, caregiver support, and more.

Prevention is a key element of a balanced community health program. The Visiting Nurse Association and Hospice of Vermont and New Hampshire offers clinics in many of the towns it serves. These clinics help people avoid illness and control chronic diseases. The monthly schedule is available on their website.
