Our Child, Adolescent and Family Services department provides individual, family and group mental health services for those ages 0-25. We also offer a substance use disorder program that is based on a specially developed program that has been successful for persons ages 12-25. We are also providing services within the schools, as part of our Mental Health in the Schools program, and have added Mental Health in the Camps services at various local Summer camps during the season. A Peer Support center providing a non-medical approach to mental health issues for adults eighteen (18) years and older. It offers: social/recreational activities, support/wellness groups, education, workshops, and more.

Presentations for community organizations, schools, social service agencies, medical and legal professionals, business owners and law enforcement are developed to raise awareness and understanding of domestic and sexual violence and stalking.

Your oncology nurse navigator is your advocate, reinforcing and supporting the physician/patient relationship. Oncology nurse navigators help newly diagnosed cancer patients, current cancer patients and family members. Your oncology nurse navigator will: Facilitate communication between all members of the healthcare team Provide education on disease processes, treatment options and clinical trial availability Help patients make informed decisions about their care Coordinate office visits across medical disciplines to streamline cancer care delivery Serve as a reliable and trusted source with answers to any questions Organize multidisciplinary team conferences to promote collaboration in cancer treatment Provide a personal touch to cancer care by visiting patients at appointments, assessing patients’ needs and relaying relevant information to providers Advocate for patients’ needs and link them with cancer support resources

Advocates help persons with disabilities obtain Social Security, SSI, and APTD benefits and access to health care through Medicare and Medicaid. The Project also works for improvements in the APTD/Medicaid system. Advocates assist individuals and families with eligibility issues for unemployment compensation, state assistance programs such as food stamps, Medicaid and cash benefits for families, and municipal welfare programs.

Services are available to everyone, regardless of age, gender, race, religion, sexual orientation, income or physical abilities. - Advocates can be reached 24 hours a day, 7 days a week to provide free and confidential services. - Prevention education programs also available to the community.

Agency providing services to survivors of domestic or sexual violence, stalking, and human trafficking in Cheshire County and western Hillsborough County. Services include peer support, legal advocacy, hospital accompaniment, support groups, police accompaniment, emergency shelter, and referrals to community partners for other needs.

The Navy's Family Readiness programs have been afforded the highest visibility, advocacy, and priority. Advocates provide emotional support and information for survivors of domestic and sexual violence and stalking through our 24-hour support line. An advocate is also available in person at the office. Advocates can meet survivors at the hospital, police department, or court as well.

Youth Activities TLC offers a variety of activities and opportunities for youth to engage in throughout the year. These activities are a way for youth to engage and interact in a safe and supportive environment.

In addition the HJP fights housing discrimination by investigating complaints of discrimination. The HJP also helps individuals and families in emergency situations who are either currently without shelter or at imminent risk of becoming homeless. The HJP handles housing cases such as: Section 8 or public housing Mobile home park issues Fair Housing/housing discrimination complaints Housing accessibility issues for persons with mobility disabilities
