Our Community Support Program provides a wide array of specialized services for eligible adults living with a severe mental illness. Individualized treatment plans are designed to achieve overall health and successful community living. Our staff has expertise in psychiatry, nursing, evidence-based programming, mental health and substance use counseling, and case management. Services are provided at our offices in Concord and Franklin, as well as in clients’ homes and in the community.

Comprehensive Outpatient Community Mental Health Services.

Our Child, Adolescent and Family Services department provides individual, family and group mental health services for those ages 0-25. We also offer a substance use disorder program that is based on a specially developed program that has been successful for persons ages 12-25. We are also providing services within the schools, as part of our Mental Health in the Schools program, and have added Mental Health in the Camps services at various local Summer camps during the season. Comprehensive Outpatient Community Mental Health Services for Adults and Children. Services may include: Case Management, Outpatient Therapy, Psychiatric Services, Functional Supports and Services, Supported Employment Services, Assertive Community Treatment (ACT), Illness Management and Recovery, Financial Case Management, Infant/Early Childhood Mental Health, Parenting Education

The Autism Center helps children, youth, young adults, and their families. The Center uses Applied Behavior Analysis (ABA). Parents receive training and strategies they can use in their natural family routines. The Center helps youth and young adults with autism and their families plan for adulthood.

Mental health services for ages 3 and older. Services include, but not limited to: individual, couples, group, family therapy and Psychological and Neuropsychological assessments and testing. Support services for established clients may include: case management, vocational, community, and home based services.

Non medical peer support for adults eighteen (18) years and older with a mental illness. Groups, activities, and educational activities are also offered. Transportation for members only when available.

Provides court advocacy, support and court accompaniment for victims of domestic violence, sexual assault, or stalking who are applying for restraining/protective orders, or who are witness or defendants in criminal cases. Advocates provide emotional support and information for survivors of domestic and sexual violence and stalking through our 24-hour support line. An advocate is also available in person at the office. Advocates can meet survivors at the hospital, police department, or court as well.

In-courthouse assistance with completing paperwork and filing for a temporary restraining order.

Peer Recovery Support Specialists promote recovery, remove barriers, build recovery capital, connect recoverees with recovery support services; and encourage hope, optimism and healthy living. Peer Recovery Support Specialists often will assist individuals with removing barriers such as assisting with employment, housing, educational needs, connections to mental health resources when desired, connections to meetings through multiple pathways as well as recovery oriented social network events, activities and other wellness related opportunities. Peer Recovery Support Specialists also provide crisis response and resources for community members of all ages who may be struggling with substance misuse and are seeking treatment and recovery options.

Individual classes for prenatal/parenting education where vouchers are earned for the materials boutique. Doctor Dad program offered for first time fathers. Other support offered as well.

Court Advocacy is provided by trained volunteers and staff who offer support and assistance in obtaining protective orders and during other court appearances. Also provided is 24 Hour Accompaniment at the hospital and police departments for victims seeking support.

Community Mental Health Center for Cheshire County - Region V. Personal counseling, care management and other MFS services are available at all of our locations throughout the Monadnock region. Some clinical services are available in Keene and Peterborough and others are provided in the community.

The Navy's Family Readiness programs have been afforded the highest visibility, advocacy, and priority. In practical terms, this has resulted in increased services to family members, increased individual assistance and consultation, more varied educational programs, and a more proactive outreach, and delivery of family support services. We assist clients with access to necessary social services by identifying resources, making referrals to partner agencies, helping to fill out forms, interpreting mail, navigating the immigration process, assisting with scheduling appointments and providing transportation.
