We offer support at education meetings, guide you to community resources, and help with the transition into adult life. Children's In Home Supports - An intensive home based program helping children with complex medical and behavioral needs. Services that are offered are personal care, service coordination, consultative services (including behavioral), respite care, and environmental modifications.

Service Coordination: We offer case management support to individuals with intellectual disabilities and brain acquired injuries. We advocate and oversee supports and services to ensure continuity of care. Hammers for Veterans provides home repairs and modifications needed to ensure safety and independence. Some examples of the services provided are: installation of wheelchair ramps, widening of doors, removal of barriers in the home that impede mobility, and others repairs that impact safety.

The Assistive Technology (AT) Program may help locate, partially fund, or provide full funding for one of a wide range of devices. These allow those with MS to function more independently in their daily lives, as well as more easily participate in recreational, educational, and vocational activities.
