Living Well with Chronic Pain: Living and coping with a chronic condition and/or chronic pain can be very challenging. We offer evidence-based self-management programs designed to give you the skills, tools and confidence you need to help take control of your chronic condition(s) and live life to the fullest. Caregivers and loved ones of those who manage chronic conditions or pain are also encouraged to participate. What is covered in these programs?

A 100 bed nursing home with 24 hour care.

New Generation Shelter is a home & supportive program for pregnant women and mothers of young children who are homeless. Besides shelter other services provided to residents include: parenting education, case management, life-skills coaching, and aftercare. The shelter also has a transitional apartment for mothers and their children. Apartment is available once the mother has proven to have a stable job or school or both. Day care is available for residents. Program can also help the mothers with the necessities for the children such as diapers, wipes, clothing etc. Residents receive 3 meals a day and snacks and those with an open case for food stamps will lose their benefits as a result.

Day activities include: caring for the farm animals and the gardens, working in the greenhouses, housekeeping, cooking/baking, clerical/office support, working in the farm stand or petting farm, arts/crafts, nature walks, and horseback riding. Transportation available within a 30 min. radius

Our mobile dental units can help you get the same care as a dentist’s office. Including exams, cleanings, X-rays, fillings, extractions, restorations and more, all done by dentists and dental hygienists. And if you use a wheelchair or have trouble walking, the mobile dental unit has ramps to help you get in and out easily.

Homeless shelter for men open year-round. In the event of extreme weather only, when other homeless drop in centers are closed, this agency will open up for the day only to anyone who is homeless.

Birchtree’s year-round special education day school in Portsmouth offers individualized instruction for students with autism in grades pre-K through 12. Birchtree’s Board Certified Behavior Analysts (BCBAs), Special Educators, Speech-Language Pathologists, and Occupational Therapists develop student programs using the principles of applied behavior analysis (ABA). Direct support staff, including Registered Behavior Technicians (RBTs), are highly trained in ABA and closely supervised by BCBAs while delivering individualized instruction.

Through the Transportation Network, DAV volunteers drive sick and disabled veterans to and from VA medical facilities for treatment.

The Ossipee location acts as a warming/cooling center for older adults and adults with disabilities during extreme weather.

Permanent supportive housing for adults with mental illness and very low income. Fellowship Housing offers a continuum of housing choices and support based upon the individual's needs. The housing offers a foundation upon which the individual can achieve and maintain a level of recovery from psychiatric symptoms in order to live in the community as a citizen, neighbor, friend, worker, etc. Fellowship Housing supplies heat, water and electricity and maintains the buildings. The renter is responsible for obtaining their own food and household supplies, and also pays for telephone and cable TV.

Through our Helping Ensure Appropriate Response to Trauma (H.E.A.R.T) program, CACs are partnering with school districts to address trauma related needs of school aged children. The H.E.A.R.T program is a collaboration between school districts and the Greater Lakes Child Advocacy Center in order (with parental permission) to inform appropriate school personnel when a child has been seen at the CAC, so that school staff can respond to the child’s needs in an appropriate, trauma informed way.

Helps homeless veterans get back on their feet by assisting them in their efforts to return to work. This employment training and placement program is for all homeless veterans who want to be gainfully employed in their communities.

Computer Program provides laptop or desktop computers for individuals with MS on limited or fixed incomes. Either a laptop computer, or a desktop computer with a monitor, keyboard and mouse, will be granted. Internet access and technical support will be the responsibility of the grant recipient.
