UPDATE 12/28/15: Starting Monday, December 28, 2015 registry identification cards will be issued by mail to qualifying patients and designated caregivers whose applications have been approved. Registry identification cards issued to qualifying patients and designated caregivers cannot be used to purchase therapeutic cannabis in New Hampshire until the ATC dispensary selected by the qualifying patient opens. Applications will be processed in the order received by DHHS. ****Applications for potentially qualifying patients and designated caregivers, medical provider certification and other important information can be found on the Therapeutic Cannabis Program page on the DHHS website at http://www.dhhs.nh.gov/oos/tcp/index.htm. This information is intended to assist in correctly completing the required applications. On November 2, 2015, the NH Department of Health and Human Services (DHHS) will begin accepting applications for the Therapeutic Cannabis Program from potentially qualifying patients and designated caregivers as part of a pre-registration period. 2. Description
This is a program administered by the NH Department of Health and Human Services. The program: 1. Issues registry identification cards to, eligible qualifying patients who have been certified by their medical provider to have a qualifying medical condition and associated symptoms. 2. Issues registry identification cards to designated caregivers who are authorized to assist qualifying patients with the therapeutic use of cannabis. 3. Issues registration certificates to a maximum of four alternative treatment centers.
