NEW HAMPSHIRE SMILES ADULT DENTAL PROGRAM | DENTAQUEST

DentaQuest provides dental insurance coverage for adults who are enrolled in NH Medicaid through the NH Smiles Adult Program. Covered services include:

• X-rays and examinations
• Cleanings
• Fillings
• Limited gum-related treatment
• Tooth extractions and other oral surgeries
• Other appropriate general services such as anesthesia
• $1500 yearly limit on dental services, with the exception of preventive services.

Removable partial and full dentures for individuals who qualify for services under the following criteria:

• Developmental Disability (DD) Medicaid Waiver
• Acquired Brain Disorder (ABD) Medicaid Waiver
• Choices for Independence (CFI) Medicaid Waiver
• Nursing facility residents
