Walk-in or order through the website https://temescalwellness.org/dover-medical-cannabis-dispensary/ To obtain a medical card: Complete application, see: http://www.dhhs.nh.gov/oos/tcp/documents/applicationpatient.pdf Medical provider must also fill out a form, see: https://www.dhhs.nh.gov/oos/tcp/documents/writtencertification.pdf You will need to send these two forms, along with a copy of proof of residence and a $50 check to the Department of Health and Human Services.
