County Navigators provide comprehensive case management and the following services:

Assist clients in identifying and organizing their support systems in the community.
Support client in the development of long term service relationships
Educate clients on how and when to ask for assistance
Connect clients with local resources and provide navigation through application process(s)

Families in need of support(s), elderly in need of support(s), those unhoused or at risk of homelessness, victims of trauma or abuse, those with developmental disabilities, those in need of services related to substance use disorder.
