Description
Clients are assessed for a wide variety of needs in order to explore appropriate options. Case Management including resource identification, development of short and long term plans including applications for appropriate public assistance programs and other needed services are provided at no cost. Comprehensive follow up is also provided to an consumer working with case managers to implement a plan.

GREATER DERRY COMMUNITY HEALTH SERVICES
Facilitates comprehensive case management and resource identification services for individuals in the service area. Services may include but not be limited to identifying eligible/affordable health coverage through public and private options, coordinating service access including information on Social Security, Medicare, SNAP, Fuel, Housing and other supportive services. Additionally manages the general assistance programs for the Towns of Derry, Hampstead and Londonderry, NH.
