Eligibility

Court-ordered children and their families who are referred through the Division of Children, Youth and Families or the Department of Juvenile Justice Services. In some cases, youth may be referred by school districts.

Mon. - Fri., 8 am - 5 pm or as appropriate to meet the family's needs. 24/7 emergency support.

Program referral form to 603-668-6260 by CPSW/JPPO or School District Superintendent.

Fees

State or School district charged daily rate. Financial reimbursement by the family may be required to the State.

Description

Permanency Solutions is a high intensity, multi-faceted program that is specially designed to serve at-risk youth. The program tailors a variety of therapeutic, social and community-based services, including foster care, to the specific needs of a child and his/her family and endeavors to create a safe, stable, positive home environment for each child.
