Referral, Education, Assistance and Prevention (REAP) is a program that is available to all older adults in the community. It is designed to help seniors take control of their lives, to live a healthy, and independent lifestyle. The program offers free, confidential counseling and educational services to help participants better understand and deal with the many life changes they encounter.

Community Partners offers a wide variety of behavioral health and developmental services across the lifespan to individuals and families. We serve those who experience emotional distress, mental illnesses, substance use disorders, developmental disabilities, chronic health needs, acquired brain disorder, as well as those who are in need of information and referral to access long-term supports and services.
