The Housing Transition Program is a subsidy program for those leaving NH Hospital or other inpatient psychiatric program to private sector housing. The program operates in the same manner as Section 8 but is for individuals with mental illness to bridge the gap between the time they exit a mental health facility and the time they obtain Section 8. Participants must get on the Section 8 waitlist.

Eligibility
*Do not currently have housing or are at risk of losing housing *Are connected with services at a local community mental health center *Are ready for discharge from an institutional care setting but lack any housing options in the community *Have or will apply for Section 8 and have been determined to be an eligible Sect 8 program participant. *Priority will be given to adults and transitional aged youth at NH Hospital or Transitional Housing Services or individuals moving out of a community residence who will be making a bed available for an individual coming out of Transitional Housing or NH Hospital. *Individuals coming out of inpatient psychiatric units will also be considered.
