To obtain a medical card:

Medical provider must also fill out a form, see: https://www.dhhs.nh.gov/oos/tcp/documents/writtencertification.pdf
You will need to send these two forms, along with a copy of proof of residence and a $50 check to the Department of Health and Human Services.
